For short-term rehabilitation, our nurse case manager coordinates smooth transitions from hospital to rehabilitation to home, including pre-admission visits and a goal setting meeting upon admission. She contacts primary care physicians to keep them informed regarding their patients and schedules post discharge appointments. “Warm handoffs” with referral sources and homecare agencies ensure good communication and more successful transitions.
